Aged Care Telehealth – Care Planning
You need to be covid-vaxxed and provide evidence of this for this role.
We’re looking for 5 FULL-TIME RN2s (Year 2 and above). Sorry – no roles for RN1s, EENs and ENs.
Full training provided.
ABOUT THE ROLE: You will be supporting our client by working through a backlog of overdue care plans for their clients. The expectation is that you will complete 2 care plans a day. You will be conducting over the phone interviews with patients who need care plans reviewed, updated, or created.
You will make outbound calls to clients to gather the information that you need, review their care plan, have a conversation around planning for care and services, what services are required how often and when, review medical history, risk management etc. Care plans will be logged using our client’s CRM system and you will refer any follow-up or clinical action required to our client’s clinical department.
ABOUT YOU: You are an RN2 or above, and while aged care experience is preferred, it’s not critical as full training will be given.
LOCATION: You must be able to attend the office in Malvern East for the first week of training. After that, it’s remote work.
HOURS: 9:00am – 5:00 pm; 7.5 hour shifts (sorry – no half days)
DAYS: Monday to Friday
START: We’re looking at starting on Monday 27 May.
TRAINING: Paid training provided with ongoing support always available. Training will be provided on the first day, however, if all compliance documentation is in order, your paid training may be offered sooner than the start date. Training will be done onsite
DURATION OF WORK: Until end of July / early August, or until the backlog is cleared. Expect 8 weeks work.
TYPE OF WORK: Short-term contract
RATES: $48.73 – $64.90 per hour dependent on year level PLUS Super.
NICE TO KNOW: We can only accept applications from nurses who can work 5 days a week. Aged Care experience is preferred but not a prerequisite, nor is experience in care planning as full paid training will provided.
